NAD+ is essential to life. It helps cells process energy and supports enzymes involved in metabolism, cellular signaling and DNA maintenance. Those facts are well established.
What is not established is whether taking an NAD+-related supplement can reverse aging, extend human lifespan or reliably make a healthy person feel younger. The science is interesting, but the marketing often moves faster than the evidence.
What Is NAD+?
NAD stands for nicotinamide adenine dinucleotide. It is a coenzyme found throughout the body, meaning it helps enzymes perform important chemical reactions.
NAD moves between two main forms, NAD+ and NADH, as it accepts and transfers electrons. This cycling helps cells convert nutrients into usable energy. NAD+ is also used by enzymes involved in DNA repair, cellular stress responses and metabolic regulation.
Calling NAD+ a “cellular energy molecule” is therefore reasonable—but incomplete. Its presence inside cells does not mean that taking more automatically produces noticeable energy or better health.
What Does NAD+ Have to Do With Aging?
Researchers have observed age-related changes in NAD+ metabolism in animals and some human tissues. These findings have generated interest in whether supporting NAD+ production could influence aspects of aging.
The biology is more complicated than the statement “NAD+ declines with age.” NAD+ levels can differ among tissues, and measurements may be influenced by health status, metabolic conditions, inflammation, lifestyle and the method used to test them.
An association also does not establish cause and effect. Lower NAD+ may contribute to certain age-related changes, result from them, or reflect several biological processes occurring together.
Why NMN and NR Receive So Much Attention
The body can build and recycle NAD+ using several starting materials. These include compounds related to vitamin B3.
Two widely discussed NAD+ precursors are:
- NMN: Nicotinamide mononucleotide
- NR: Nicotinamide riboside
A precursor is a substance the body can use within a pathway to make another compound. Researchers study NMN and NR because oral NAD+ itself must pass through digestion and metabolism, while precursor compounds may enter NAD-related pathways differently.
NMN, NR, nicotinamide and NAD+ are related, but they are not interchangeable. Evidence about one ingredient should not automatically be applied to another product or delivery method.
What Human Studies Have Found
Human trials suggest that some NMN and NR preparations can alter NAD+-related measurements in blood or other tested samples. This demonstrates biological activity, but it does not answer the larger question of whether those changes consistently improve health.
A 2025 systematic review and meta-analysis of 12 NMN trials involving 513 participants found that supplementation increased blood NAD levels. However, most clinically relevant metabolic outcomes were not significantly different from the control groups, and the researchers identified concerns about bias within the included studies.
Research examining physical function has also produced mixed results. A 2025 review concluded that current evidence does not support using NMN or NR to preserve muscle mass or function in adults with an average age above 60.
Other small trials have reported potentially encouraging findings involving selected measurements, but the results are not uniform. Differences in study population, ingredient, amount, duration and outcome make broad conclusions difficult.
A Better Biomarker Does Not Always Mean a Better Outcome
A biomarker is a measurable biological signal, such as the concentration of a substance in blood. Biomarkers are valuable research tools, but changing one does not automatically mean that a person will feel better, remain healthier or live longer.
This is one of the most important distinctions in NAD+ research:
- A product may increase an NAD-related laboratory measurement.
- That increase may confirm that the compound entered a biological pathway.
- It does not automatically prove better energy, cognition, strength, disease prevention or longevity.
Meaningful health outcomes require direct evidence. They cannot be assumed from a laboratory marker alone.
What Has Not Been Established
Current human evidence does not establish that NAD+, NMN or NR supplements:
- Reverse biological aging
- Extend human lifespan
- Prevent age-related disease
- Reliably increase everyday energy in healthy adults
- Restore youthfulness
- Produce the same results as those reported in animal studies
Animal and laboratory research can help researchers understand mechanisms and identify promising questions. It cannot, by itself, demonstrate that the same intervention will produce a meaningful or safe result in people.
What About NAD+ IVs and Injections?
Intravenous and injectable NAD+ services are marketed separately from oral supplements. Bypassing digestion changes how a substance enters the body, but a more direct route does not automatically prove a better health outcome.
Published clinical evidence supporting broad anti-aging, energy or longevity claims for injected or infused NAD+ remains limited. Sterility and compounding quality also introduce risks that do not apply in the same way to ordinary oral products.
The FDA has reported adverse events associated with certain compounded injectable NAD+ products and has warned compounders about the risks of using ingredients unsuitable for sterile drugs. Injectable or intravenous products should only be considered with an appropriately qualified healthcare professional.
What Do We Know About Safety?
Several short-term human studies of specific NMN and NR preparations have reported that participants generally tolerated the tested products. That does not establish long-term safety, universal suitability or safety at amounts outside those studied.
The research often involves relatively small groups and short study periods. Products can also vary in identity, purity, formulation and actual ingredient content.
Discuss NAD+-related supplements with a qualified healthcare professional if you take medications, have a medical condition, are pregnant or breastfeeding, are preparing for surgery or are receiving treatment for cancer or another serious illness.
Follow the product directions. Do not assume that a larger amount will produce a greater benefit, and do not combine several NAD-related ingredients without reviewing the complete routine.
How to Evaluate an NAD+ Product
If you are considering the category, start with questions rather than promises:
- What ingredient does the product actually contain: NAD+, NR, NMN or another form of vitamin B3?
- Does the Supplement Facts panel disclose the amount per serving?
- Is the marketing based on human outcomes or mainly on laboratory and animal research?
- Does the cited study examine the same ingredient and a comparable formulation?
- Is the promised benefit more ambitious than the evidence?
- Could the product overlap with other supplements or medications?
- How will you evaluate whether it is useful?
Be cautious when a product promises age reversal, guaranteed energy, cellular repair, disease prevention or a longer lifespan. Those claims extend beyond what current human evidence can support.
The Foundation of Healthy Aging Has Not Changed
NAD+ research may eventually lead to useful applications. That possibility does not replace the health practices already supported by a much larger body of human evidence.
Healthy aging still depends heavily on:
- Regular physical activity, including strength and aerobic movement
- A varied and balanced eating pattern
- Consistent, sufficient sleep
- Preventive healthcare and appropriate screening
- Managing blood pressure, blood sugar and other established risk factors
- Maintaining social connection and mental engagement
- Avoiding tobacco and limiting harmful alcohol use
Supplements, when appropriate, remain optional tools within that larger foundation.
The Bottom Line
NAD+ is essential to cellular metabolism, and research into its relationship with aging is scientifically meaningful. Human studies show that some precursor supplements can change NAD-related biomarkers, but meaningful benefits for energy, physical function, healthy aging and lifespan remain uncertain.
The honest position sits between dismissal and hype: the biology is real, the research is active, and the strongest anti-aging promises have not been proven.
